SPECIAL NOTES TO FAMILIES/CAREGIVERS: We strongly encourage ALL families/caregivers to submit a Free & Reduced Meal Application (accessed at left OR online through Family Access in Skyward) - whether or not you feel there is a need for meal benefits for your family or if you would qualify. This form is used to provide access to a multitude of benefits for your student.

Completing the application takes less than 10 minutes and families/caregivers only need to complete one meal application for their entire family. Once you submit an application, you will receive a notice by mail whether your student(s) can receive free or reduced meals and/or other benefits.

If you would like your student to have access to the additional benefits outlined above following completion of your Free & Reduced Meals Application, completing the Consent to Release Information Form (linked at left) is also REQUIRED. Without this accompanying form, your student(s) will only have access to the free and reduced meal price benefit.

USDA Nondiscrimination Statement

To file a program discrimination complaint, a Complainant should complete Form AD-3027, USDA Program Discrimination Complaint Form(link is external), from any USDA office, by calling 866-632-9992 or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
